How to Apply


Please determine the eligibility (retroactive or current) prior to starting the application process.


  • Part A must be completed, printed and signed by the Marine or legal guardian. Part A must be mailed, faxed or scanned to HQMC along with part B in order for the claim process to be initiated.
  • Part B must be completed and signed by the Marine’s physician. Part B must be mailed, faxed or scanned to HQMC along with part A in order for the claim process to be initiated.

  • Once these forms have been completed TSGLI Claims can be submitted in ONE of three ways. Please only use one of the options below:

    Important Information for Physicians:

    Please read the definition for "other traumatic injuries" located under FAQ's.


  • Submit medical documentation that supports the loss claimed in Part B.
  • Part C will be certified by HQMC once A and B have been received and the information is verified. Once CERTIFIED by HQMC, the claim will be forwarded to the Office of SGLI for determination.
