How to Apply
Please determine the eligibility (retroactive or current) prior to starting the application process.
- Part A must be completed, printed and signed by the Marine or legal guardian. Part A must be mailed, faxed or scanned to HQMC along with part B in order for the claim process to be initiated.
- Part B must be completed and signed by the Marine’s physician. Part B must be mailed, faxed or scanned to HQMC along with part A in order for the claim process to be initiated.
Once these forms have been completed TSGLI Claims can be submitted in ONE of three ways. Please only use one of the options below:

Important Information for Physicians:
Please read the definition for "other traumatic injuries" located under FAQ's.
- Submit medical documentation that supports the loss claimed in Part B.
- Part C will be certified by HQMC once A and B have been received and the information is verified. Once CERTIFIED by HQMC, the claim will be forwarded to the Office of SGLI for determination.
